NEW PATIENT CHECKLIST
Payment policy:
We require all patients to pay their deductible, copay and/or coinsurance payment at the beginning of each visit. At the conclusion of your visits with us, you may be billed for any outstanding balances. If there is a credit, you will be refunded promptly.
Insurance:
Although we are contracted with most insurance carriers, our services may not be covered by your particular insurance plan. Being referred to our clinic by another physician does not necessarily guarantee that your insurance will cover our services. Please remember that you are 100% responsible for all charges incurred: your physician's referral and our verification of your insurance benefits are not a guarantee of payment. We highly recommend you also contact your insurance carrier and check your coverage for a cardiology visit. Our in office and telemedicine visits are all billed as office visits and considered specialist visits. Do not assume that you will not owe anything if you have more than one insurance policy.
Missed Appointments:
We adhere to a strict cancellation policy. You are required to provide our office at least 24 business hours notice of cancellation of all visits and procedures at 804-530-1044. If your appointment is on a Monday, you must notify us of your cancellation by your scheduled appointment time on the preceding Friday. This will allow us to contact patients from our waiting list. You will be charged a fee of $35.00 if you do not cancel with at least 24 business hours notice or if you do not show up for your appointment.
Missed Nuclear Appointments:
If procedure preparation is not followed strictly, you fail to provide 24 business hours cancellation, or you do not show up for your appointment, a $150 - $175 medication fee will be charged to you since each medication is ordered specifically for you.
*If you need a referral, please make sure you have it before your visit. If you need one and don’t provide one you will be responsible for complete payment of your visit. If you have any questions please contact our office at 804-530-1044.
