What to expect at your first appointment with Dr. Morris

Please bring the following to your appointment:

  • All current Insurance Cards 

  • Identification

  • All Medications

  • Referral* - if necessary

  • Co-Payment - per your insurance agreement

  • *If you need a referral, please make sure you have it before your visit.  If you need one and don’t provide one you will be responsible for complete payment of your visit.
