BILLING & INSURANCE INFORMATION

Morris Cardiovascular and Risk Reduction Center participates with most major insurance plans. As a courtesy to our patients, our staff will automatically file your insurance claims. Please provide us with all necessary insurance data,

including the name and address of your company and your policy number.

2023 Financial and Cancellation Policies

Copayments: All co-payments will be collected at the time of service. The appointment will be rescheduled if the copayment cannot be met.

Self-Pay and High Deductible Health Plans: For self-pay and high deductible health plan members,  Morris Cardiovascular will collect at time of service for an office visit OR testing unless the patient has met their deductible for the year. Receipts will be mailed to patients.

Missed Appointments and Late Cancellations: Missed appointments or cancellations made less than 24 hours prior to the scheduled appointment time may result in a cancellation fee of $35 for standard appointments, $100 for Echo, Vascular testing, and outpatient procedures, and $150 for Nuclear stress testing. After the third consecutive no show/late cancellation for a specific test or appointment, a subsequent test/appointment will not be scheduled until a $200 penalty is paid.

Form Completion: There will be a $20 pre-payment charge for all forms that need to be completed by a Dr. Morris or any of the providers at Morris Cardiovascular and Risk Reduction Center. Please allow 7 business days for form completion. Please fill out any patient-specific sections prior to submission. All forms need to include instructions as to where to send the forms once completed, along with a self-addressed stamped envelope if they are to be mailed.
